McCormick Academy

The participant and parent/guardian acknowledge that hockey and related activities involve inherent risks, including injury, and voluntarily assume all such risks. They release and hold harmless McCormick Academy, its coaches, owners, shareholders, staff, and affiliates from any claims for injury, illness, accident, or loss of personal property. The parent/guardian authorizes emergency medical treatment for the participant and accepts responsibility for any related costs. They also grant permission for McCormick Academy to use photos or videos of the participant for promotional purposes without compensation. By signing below or completing registration online, the participant and parent/guardian confirm they have read, understand, and agree to these terms.
